Purpose:
|
To compare one year results of total versus partial epithelium removal in corneal collagen cross linking (CXL) in the treatment of progressive keratoconus.
|
Methods:
|
This study retrospectively compared results of total (group T) versus partial (group P) approaches of epithelium removal in CXL. Changes in vision parameters, refraction, and Pentacam indices were compared up to one year after CXL.
|
Results:
|
At 1 year after CXL, uncorrected visual acuity (UCVA) in groups T and P improved by 0.13±0.42 and 0.12+0.36 logMAR (P=0.447), respectively, and best corrected visual acuity improved by 0.00±0.19 and 0.13±0.20 logMAR (P=0.001). Spherical equivalent decreased by 0.44±1.25 diopter (D) in group T and 0.56±1.47D in group P (P=0.054). The decrease in maximum keratometry was 0.39±0.93D and 0.01±0.95D in groups T and P, respectively (P=0.037) and the decrease in mean keratometry was 0.42±0.93 and 0.00±0.65D (p=0.015). Central corneal thickness decreased by 18.39±20.66µm in group T and 0.11±13.29µm in group P (P<0.001).

Conclusion:
|
According to one year CXL results, both approaches have similar results in terms of improved UCVA. With total removal, better corneal flattening is achieved. Long term studies would be helpful to compare these two approaches in terms of result stability and impact in stopping the progression of keratoconus.
