In imaging study superior orbital cystic lesion with bone erosion and contrast enhancement was noted. Incisional biopsy of cyst wall showed xanthogranulomatus inflammation with no fungal and acid fast element. Related laboratory tests for rheumatologic and infectious disease were normal. She received Intravenous dexamethasone 4mg/TDS 24 hours then changed to prednisolone 1 mg/kg/d continued for 4 weeks then tapered over 2 weeks. Proptosis and ptosis were improved gradually through 2 weeks. No recurrency was seen through 5 months.
