Purpose:
A case report of superior orbital xanthogranuloma successfully treated with corticosteroid.
Methods:
The patient is a 15 years old lady who presenting to Labafinejad clinic for eyelid ptosis in left side. Related laboratory and imaging studies and incisional biopsy were performed then outcomes of treatment were evaluated.
Results:
At presentation there were no other known diseases. Her vision was complete. Ptosis 2mm and proptosis 3mm in left eye was measured. SLE and dilated fundoscopy revealed no abnormality. In imaging study superior orbital cystic lesion with bone erosion and contrast enhancement was noted. Incisional biopsy of cyst wall showed xanthogranulomatus inflammation with no fungal and acid fast element. Related laboratory tests for rheumatologic and infectious disease were normal. She received Intravenous dexamethasone 4mg/TDS 24 hours then changed to prednisolone 1 mg/kg/d continued for 4 weeks then tapered over 2 weeks. Proptosis and ptosis were improved gradually through 2 weeks. No recurrency was seen through 5 months.
Conclusion:
Adult orbital xanthogranulomatus is a rare disease. Radiologic signs of this mass were similar to a malignant tumor. Incisional biopsy confirmed the diagnosis and helped for choosing an appropriate treatment modality.
