Purpose:
To compare the 18 month-results of accelerated and standard collagen cross-linking (CXL) in the treatment of progressive keratoconus.
Methods:
Thirty-one patients (31 eyes in intervention and control groups) were enrolled. The intervention group received accelerated (18mW/cm2, 5min) and control group received standard CXL (3mW/cm2, 30min).
Results:
The mean changes of uncorrected visual acuity (P=0.745), best corrected visual acuity (P=0.551), refractive astigmatism (P=0.823), and manifest refraction spherical equivalent (P=0.178) did not differ significantly between the two groups. The central corneal thickness was higher in the standard group than the accelerated group (P=0.324). The mean decrease of the maximum-K (P=0.039) and mean-K (P=0.051) in standard group were more than in accelerated group. Mean changes of asphericity (P=0.426), corneal hysteresis (P=0.983) and corneal resistance factor (P=0.596) were similar in both groups. The mean decrease of the ECC did not show a significant difference between the two groups, as well (P=0.405).
Conclusion:
Based on the eighteen-month results, similar to the standard method, accelerated CXL arrests the progression of the disease. The corneal flattening was more than obtained using standard CXL.
