Purpose:
This study was conducted to evaluate the efficacy of combined endoscopic cyclophotocoagulation (ECP), phacoemulsification (PE), and pars plana vitrectomy (PPV) on the intraocular pressure (IOP) of patients with diabetic neovascular glaucoma (NVG).
Methods:
Nine phakic eyes of 8 patients with advanced diabetic retinopathy, neovascular glaucoma and uncontrolled IOP were entered in this study. Twenty gauge PPV combined with phacoemulsification, intraocular lens implantation, 270º degree ECP, and intravitreal bevacizumab was done for all the patients. Best corrected visual acuity (BCVA), IOP and the number of antiglaucoma medications were evaluated 1, 3, 6, and 12 months after the procedure
Results:
The mean preoperative IOP was 45±11.29 mmHg and statistically significantly decreased to 19.22±8.95 mmHg, 21.33±13.46 mmHg, 17.50±1.51 mmHg, and 18.00±1.09 mmHg 1, 3, 6, and 12 months after the operation, respectively (P=0.008, P=0.008, P=0.012, and P=0.027, respectively). The mean preoperative logarithm of the minimum angle of resolution BCVA improved from 1.53±0.31 at baseline to 1.17±0.09 at month 12 (P=0.046). The mean number of antiglaucoma medication was statistically significantly decreased.
Conclusion:
Combination of ECP and PE with standard PPV may improve the visual acuity and control IOP in patients with Phakic diabetic NVG.
