Purpose:
To present an unusual case of chronic angle-closure glaucoma who was incidentally diagnosed with Adie's tonic pupil and plateau iris configuration.
Methods:
With a provisional diagnosis of Adie's pupil, systemic work up and careful physical examination were performed and all were negative regarding systemic involvement and secondary ethiologies of the condition.Pilocarpin 0.125% test confirmed the diagnosis. The patient underwent phacotrabeculectomy and simultaneous viscogoniosynechiolysis.
Results:
Untill two months after surgery IOP was within normal limit without antiglaucoma medication and vision did not deteriorate.
Conclusion:
Ophthalmologist must consider the possibility of angle-closure glaucoma in a patient with Adie’s pupil, otherwise similar cases may be underdiagnosed.
