Isolated bilaterl optic neuritis is an extremely rare manifestation of Behçet syndrome.

case report

Results:
A previously healthy 28-year-old white man complained of acute headache and mildly blurred vision since a month earlier which did not respond to analgesics. Brain MRI was normal. At the next visit (a month later), the ophthalmic examination showed the best corrective vision was 7/10 1n both eyes and 2+cell in the righ eye (AC). Except for leukocytosis and positive CRP other test(ICP and MRV) were normal. Rheumathology consultation revealed a history of Behcet in the patient and his brother and HLA B5 was positive. Optic discs swelling significantly decreased with IV-steroid and headache disappeared. Two weeks later BCVA was 9/10 and papillitis resolved.
Conclusion:
Papilledma in the behcet is common due to cerebral venous thrombosis but bilateral papillitis is rare.In the patient CSF, ICP and MRV were within normal limits and symptoms and signs did not decrease with LP and oral Diamox. Then papilledema did not consider for diagnosis and cerebral venous thrombosis was excluded.
