Purpose:
|
To investigate the long term intraocular pressure (IOP) changes and incidence of glaucoma after pars plana vitrectomy.
|
Methods:
|
In this historical cohort study, patients with history of vitrectomy on one eye who met the inclusion/exclusion criteria were enrolled. More than 6mmHg rise in IOP was considered significant. Patients who needed glaucoma medication/surgery or had increment of more than 0.2 in cup/disc ratio were considered to have developed open angle glaucoma (OAG).

Results:

Two hundred and thirty nine patients were enrolled. Mean follow up was 20.6 (3-96) months. Mean baseline IOP was 13.5mmHg and mean final IOP was 16mmHg. Significant IOP rise occurred in 37 study eyes (15%) and 7 fellow eyes (3%, P<0.001). Post operative lens status (p=0.857), indication of vitrectomy (p=0.301) and number of operations (p=0.348) were not associated with IOP rise. Silicon oil injection showed a tendency to rise IOP (P=0.050). OAG occurred in 17 study eyes (8%) while none of the fellow eyes developed glaucoma. Eyes with more than 1 vitrectomies showed a higher incidence of OAG (P<0.0001.

Conclusion:
|
Vitrectomy can increase the risk of IOP rise which is not affected by SO injection, post operative lens status, and indication of vitrectomy. Repeated vitrectomies are a risk factor for OAG.
