Purpose:
To describe a case of bilateral cytomegalovirus retinitis (CMVR) in an otherwise healthy infant
Methods:
A 4-month-old male infant with history of fever and visual inattention since 1 week before admission was referred for eye examination
Results:
Ocular involvement was detected as bilateral panuveitis with diffuse bilateral retinitis and vaculitis both in the peripheral retina and posterior pole . Fundus photographs were made with a RetCam 120 wide-field digital fundus camera.
Past medical and ocular history in the patient were unremarkable . All of the laboratory tests for immune systems and systemic work-up were normal except for CMV infection. His mother had history of fever and malaise during 8 months of pregnancy with unknown diagnosis. Regarding to PCR detection of CMV DNA in aqueous and CSF(cerebro-spinal fluid) and positive serum IgG in his mother , diagnosis of bilateral CMVR in an immunocompetent infant possibly due to maternal transmission was made.
Conclusion:
Peri-natally acquired CMV infection in an immunocompetent infant can be potentially a fatal disease.
