Purpose:

To report a case of pseudoexfoliation syndrome in a young patient following penetrating keratoplasty.

A 38 year old woman underwent penetrating keratoplasty(regraft) in her right eye due to graft failure. Her first graft was done 25 years ago due to keratoconus in Shiraz.We noted pseudoexfoliative material (PXM) over pupil margin and anterior lens surface of operated eye 11 months after operation(2nd graft).Other ophthalmic examinations were normal. The donor was a 40 year old man who died because of car accident. Sutures were removed 14-18 months after surgery. The patient was followed for 8 years. Intraocular pressure was16-17mmHg in both eyes uncorrected visual acuity was 20/40 in operated eye. The fellow eye showed mild keratoconus and PXM was not found at the end of follow up.

Conclusion:
|
Psedoexfoliation syndrome can be triggered to occur by intraocular surgery in susceptible patients.
