Purpose:
|
To report a unique case of late iatrogenic bleb formation and macular hypotonia after pterygium surgery
|

The patient had a history of pterygium surgery 4 years ago, with a combination of conjunctival autograft and adjuvant intraoperative mitomycin C 0.02%. In slit lamp biomicroscopy, in the right eye, an elevated, bleb-like lesion was noticeable.The sclera seemed fistulated at the site and a thin layer of conjunctiva completely covered the lesion and marked choroidal folds in macular region was obvious.
Donor sclera patch graft was secured over the fistula with sutures but one month later, a small bleb re-appeared adjacent to the scleral patch graft. IOP was 2 mmHg, argon-laser treatment of the bleb was tried to induce scarring. The procedure was highly effective and after one week the IOP raised to 8 mmHg.The clinical features remained stable through all other visits, until last follow-up, 12 months after initial presentation.

Conclusion:

This is the first reported case of a late iatrogenic bleb formation after pterygium surgery using MMC adjuvant and consequent macular hypotonia.
