Purpose:

To report a three chamber cardiac myxoma, presented with neuro-ophthalmologic symptoms
|

We present a case of three myxoma in right atrium (RA), left atrium (LA) and left ventricle (LV) presented with symptoms of cerebellar ischemia.
A 24 year old male was admitted to neurology department because of severe vertigo; visual and equilibrium disturbances and syncope. Cardiac MRI showed multiple small infarctions in posterior segments of left hemisphere and cerebellum. Patient was referred for a Transthorasic exam (TTE) revealed three separated masses in RA, LA and LV. Atrial masses were highly mobile and cause significant functional stenosis of mitral and tricuspid valves.
Cardiac surgery was performed in an urgent basis, masses were removed by a longitudinal left atriotomy .Pathologic exam report, confirmed three masses to be primary cardiac myxoma.
Patient was discharged 11 days after and experienced neither new symptoms nor relapse of cardiac myxoma by TTE for 8 months.

Conclusion:

to seek more attention to more unusual presentations of intra cardiac pathologies and emphasize on value of team work
