Purpose:
|
Spontaneous retrobulbar hemorrhage(SRH)is a rare but sight threatening incident. We report a very rare case of SRH in an acute myeloid leukemic(AML)patient for first time.
|
Methods:
|
A 75-year-old man was referred to the ophthalmology service for sudden onset of proptosis since 10 days ago. Examination revealed tense proptosis, periorbital edema, ecchymosis, and limitation in ocular motility and visual acuity of NLP.He is known case of AML.Blood cell count revealed a marked thrombocytopenia with 12000/microliter.We consider canthotomy and platelet increasing.

Results:
|
For differential diagnosis in AML patients with proptosis, SRH, must taken into account. Hemorrhagic ocular complications may be associated with thrombocytpenia. Possible reason of poor visual prognosis in present case is referral delaying (about 10 days).

Conclusion:
|
The clinician should maintain a high index of suspicion in identifying SRH, when managing patients with blood dyscrasias and early refer and treatment is necessary.
