Purpose:
|
To determine and compare the short-term visual function of combined intravenous erythropoietin and methylprednisolone with the standard intravenous methylprednisolone treatment in patients with acute isolated retrobulbar optic neuritis(IRON).

Methods:
|
Adult patients with IRON and best corrected visual acuity (BCVA) less than 20/40 were recruited. Patients were assigned to combined intravenous erythropoietin as adds on to the standard intravenous methylprednisolone(group1),only standard intravenous methylprednisolone(group2).BCVA,contrast sensitivity,color vision and visual field,were assessed before treatment and up to120 days treatment.

Results:

Sixty two patients with IRON were assigned to group1 (n=35), and group2(n=27), respectively.Mean age of patients was 26.6.BCVA 3months after treatment was 0.19±0.55 and 0.11±0.32 logMAR in group1 and group2 respectively(P= 0.51).BCVA of 20/20 or better was achieved in 29(71.4%) patients in group1,in 19 (59.3%) patients in group2.Patients group1 showed faster recovery of BCVA,but it did not reach to the significance level.

Conclusion:
|
Intravenous human recombinant erythropoietin as add-on to standard methylprednisolone treatment could not statistically improve short-term visual function results compared to systemic high dose methylprednisolone in isolated acute retrobulbar optic neuritis.However,the combination of erythropoietin with methylprednisolone showed a consistently superior result in magnitude and pace of visual functions recovery that could justify further clinical trials.
